篩骨洞に原発した腺房細胞癌の1例

A Case of Acinic Cell Carcinoma in Ethmoid Sinus

I．はじめに

　腺房細胞癌（acinic cell carcinoma）は一般に唾液腺に発生する腫瘍で，悪性腫瘍の中では比較的予後のよいものである。発生部位は大多数が耳下腺であり，そのほかでは口蓋，口唇，臼後部などに発生したとの報告がみられる。本腫瘍が鼻副鼻腔に発生することはきわめてまれであり，著者らの渉猟しえた範囲では本邦には篩骨洞に発生したという報告はない。われわれは右篩骨洞に発生した腺房細胞癌症例を経験したので，若干の文献的考察を加えて報告する。

　A 60-year-old man complained of nasal bleeding for four months. A CT scan showed a markedly enhancing mass in the right ethmoid sinus and maxillary sinus. The tumor was very hemorrhagic, and about a half of the tumor was removed with bilateral external rhinotomy in the first operation. Irradiation therapy was performed postoperatively, and the residual tumor was removed completely in the second operation. Histological examination revealed acinic cell carcinoma.

　This case showed a good clinical course with-out recurrence and metastasis.
