はじめに

　わが国における悪性軟部腫瘍の発生率は10万人に2人で，剖検数からみた全悪性腫瘍に占める頻度は0.14％であった^1）。日本整形外科学会の全国骨軟部腫瘍登録一覧表（2010年）によると，脂肪肉腫の組織別発生頻度は最も高く38.4％であった^2）。一方，頭頸部における脂肪肉腫の発生は稀であり全肉腫中の約1％^3），全脂肪肉腫中の3.4〜4.4％と報告されている^4,5）。さらに，耳下腺発生の詳細な症例報告はわれわれが渉猟した限りわが国では認められず，国外では6症例がみられたのみであった^6〜8）。

　今回われわれは，中年男性の耳下腺に発生した高分化型脂肪肉腫の1症例を経験したので，若干の文献的考察を加えて報告する。

　Liposarcomas are rare malignant mesenchymal neoplasms in the head and neck region, especially in the parotid gland. Based on a thorough search English literatures, there were only 6 cases reported.

　A 47-year-old man presented with a pre-auricular tumor in the right side and subsequently he underwent the right partial parotidectomy. The histopathological findings revealed the presence of atypical and hyperchromatic stromal cells, and varying number of mono- or multi-vacuolated lipoblasts. Finally the diagnosis was made as well differentiated liposarcoma. There was no recurrence in 5 years postoperatively.
