耳下腺腫瘍における穿刺吸引細胞診の臨床統計的検討

A clinico-statistical study in parotid tumor fine-needle aspiration cytology

はじめに

　穿刺吸引細胞診（fine-needle aspiration cytology：FNAC）は，耳下腺腫瘍の術前検査として古くは播種の危惧から禁忌とされていた。しかし，1980年初頭より徐々に行われるようになり^1），現在は一般的な検査となっている。当科では，筆者が赴任した1988年からエコーガイド下にほぼ全症例に行ってきた。

　今回，手術を行った耳下腺腫瘍一次症例の術前FNACの結果と術後病理組織診断の結果を統計的に解析・検討し，当院におけるFNACの有用性を明確にし，さらに精緻な術前検査とすることが本報告の目的である。

　Fine-needle aspiration cytology（FNAC）had been undergone with ultrasonography since 1988 in our hospital, as an indispensable tool for the initial diagnosis in head and neck tumors.

　In the present study we analyzed the efficacy of FNAC as a pre-operative examination on 167 cases of parotid tumors, which comprised 139 with benign tumors and 28 with malignant tumors. The results of cytological examinations from FNAC delivered an accuracy of 91.6％, a sensitivity of 53.6％, a specificity of 99.3％, and a predictive value rate for a specific histologic diagnosis was 93.9％ in all cases.
