MRIにて非典型像を呈した鼓室型グロームス腫瘍例

Glomus tympanicum tumor with atypical imaging of MRI：A case report

はじめに

　グロームス腫瘍は中耳に発生する良性腫瘍の1つである。確定診断は病理組織診断によるが，症状・鼓膜所見に加え，画像検査が診断の重要な手がかりとなる。また血流豊富な腫瘍であり，摘出に出血のリスクを伴うため，術前に腫瘍の血流・進展範囲を正確に評価する必要がある。今回われわれはMRIにて非典型像を呈した鼓室型グロームス腫瘍の1例を経験したので，若干の文献的考察を加えて報告する。

　Glomus tumor is slow-growing benign tumor in the middle ear. We reported a patient with glomus tympanicum tumor. She complained of tinnitus in the right ear. Otoscopy examination revealed a reddish and pulsative mass in the right tympanic cavity. A glomus tumor was suspected. CT and MRI showed a tumorous shadow limited in the tympanic cavity without expanding into tympanomastoid compartment. The mass was enhanced partially in MRI imaging. The tumor was removed by transcanal tympanotomy without embolization. Histological and immune-histochemical study confirmed the diagnosis. Symptoms disappeared in a few days following the operation.
