はじめに

　良性発作性頭位めまい症（benign paroxysmal positional vertigo：BPPV）は，ある一定の頭位をとることによりめまいをきたす疾患で，現在では卵形囊から脱落した耳石がその病態に関与しており，半規管のクプラに耳石が付着するものをクプラ結石症，半規管内に浮遊耳石が移動するものを半規管結石症として理解されている。またBPPVに関与する半規管は後半規管が最も多いが，外側半規管によるものも考えられている。一般的には特発性で，通常は聴力が正常もしくは左右差を見いだせない場合が多い。しかしながら一側性感音難聴や聴力の左右差を伴った症例に少なからず遭遇する。

　今回，われわれはめまいを主訴に来院し外側半規管型BPPVと診断した症例のなかで，すでに純音聴力閾値の左右差を伴っていた症例を検討したので報告する。

　The horizontal semicircular canal type benign positional vertigo（hBPV）usually shows symmetrical pure tone hearing level, but in the transient geotropic type hBPV（canalolithiasis）, 4/14 patients had asymmetry of pure tone audiometry（PTA）and the all affected side diagnosed by nystagmus pattern coincided with the deteriorated side of PTA. On the contrary, in the heavy or light cupula type hBPV, 6/18 patients had asymmetry of PTA and the affected side did not always coincide with the deteriorated side of PTA. This outcome was thought to be derived from the different pathophysiological cause existing between canalolithiasis and cupular lesion.
