Menièreの原著とその周辺 第二編 Menièreの主論文（1）

第二編の概要

　Menièreの主論文を中心とする。第一部として公開論文朗読講演（抄），第二部として，主論文（ただし長論文であるので紙数の都合上第二編では前半を対象として，後半は第三編に掲載する），第三部として主論文に掲載して，1948年にMenièreが翻訳したKramerの耳科学書の訳者追補の中に掲載した同一剖検例の記載を比較検討する。第一部と第二部の関連について，当時の制度を解説する。1831年創刊の「Gazette médicale de Paris；Journal de médecine et des sciences accesseoires（Gaz. méd. と略称）」は当時のフランス医学界で唯一に近い権威のある雑誌であった。査読を受ける正規な論文は掲載の以前に公開論文朗読講演（procés-verbal）で全文を読み上げて，論文を審査委員会に提出する。その後に審査を経て掲載される。Menièreの場合は公開論文朗読講演は1861年1月8日であり，掲載は同年9月12日であった。しかし発表への反応は早く1月8日の翌週には優先権主張の投書があった。Menière主論文に対する反応は9月12日以前の早期のものと，さらに，それ以降19世紀に分布するものとがある。これらについては検討は終了しているので，機会をみて追加発表を行いたい。Menièreは1月8日から9月12日の間に別個の4編を投稿した。Gaz. méd. の投稿論文の取り扱いと内容からすると，三編は主論文に対する症例の追加であって症例報告である。残りの一編は学問的に論争中であったTrousseauの講演に対する見聞録であって，批判とMenièreの自説の展開がある。したがって，厳密に分類を行うとMenièreのMenière病に関する業績は主論文1編，症例報告3編，講演見聞録1編の合計で5編である。この中で後世に引用されるのは主論文のみである。以上のMenièreの全5編を他国語に翻訳したものには，Blumenbach（1955）とAtkinson（1961）の独語訳と英（米）語訳とがある。Blumenbachの独語訳はやや難解であるが，仏文としては元々やや難解なMenière特有な婉曲な文章の趣あるニュアンスを写し取ったものである。Atkinsonの米語訳は平易でわかりやすく，明るく簡明な翻訳である。今回の拙訳は2者の何れにも及ばず，なんとか和文になった程度のものである。ニュアンスとしてはBlumenbachに近く翻訳したつもりであるが，解釈には多少の異同がある。大方の批判を仰ぎたい。

　The verbal presentation,which was referred to the committee,was read at the meeting of the Akademy on January 8,1861 under the title as follows；A particular type of grave hearingloss depending on a lesion of the inner ear. On September 21,1861,the same content was published in Gaz. méd. série3 16：597-601,under the title as follows：Upon the lesions of the inner ear giving rise to the symptoms of apoplectic cerebral congestion. The change of the titles possiblly due to the discussions with Trousseau during the period. The divisin of the paper into two halves is only for the editorial reason. The verbal presentation has nothing to add except for the complaint of priority by Deleau（jeune）on January 16,which was later rejected.

　The first half of the main paper is for the introduction of Menière' disease,describing in detail the symptoms,stressing the appearance of peculier hearingloss. The major point for the discussions is the description of the findings of autopsy,of which the first report is seen in the the textbook of Kramer's translated by Menière in 1848. The second report differs in the labyrinthine lesions much conforming to the experimental lesions by Flourens.
