自己血清点耳液を用いた鼓膜穿孔閉鎖術の検討

Autologous serum ear drop therapy（ASET）for closing tympanic membrane perforations

Ⅰ．はじめに

　慢性中耳炎や鼓膜換気チューブ抜去後の鼓膜穿孔は日常診療でよく遭遇する。日帰り手術も可能なことからフィブリン糊を用いた接着法での鼓膜形成術が普及しているが，皮膚切開に心理的抵抗を感じ，外科的治療をためらう患者もいる。また，鼓膜形成術，鼓室形成術後の筋膜のずれなどで生じる小さな穿孔で術者から患者に再手術を言い出しにくい場合もある。2008年にKakehataら^1,2）によって報告された自己血清点耳液を用いた鼓膜穿孔閉鎖術（autologous serum eardrops therapy：ASET）は，外来通院で可能な皮膚切開を必要としない鼓膜穿孔閉鎖法である。簡便で非侵襲的な方法であるにもかかわらず高率に鼓膜閉鎖を得ることができる。当科では2008年よりKakehataら^1）の方法に準じてASETを行い，良好な結果を得られたので報告する。

　Autologous serum ear drop therapy（ASET）is the maneuver which was reported by Kakehata in 2008 for closing perforations of the tympanic membrane（TM）. From December 2008 to November 2010,ASET was applied in 35 TM perforation subjects at outpatient clinic. Closure of the TM perforation was achieved 29（82％）of 35 ears and reduction of the perforation size was observed in 3 ears（9％）. Closure rate for small,medium,and large perforations were 92％（23 of 25）,75％（6 of 8）,and 0％（0 of 2）,respectively.

　The result suggests that ASET is effective in closing small size TM perforations.

　Since the advantages of ASET are allergy free, no risk of possible unknown viral infection and minimum invasion, this procedure is suitable for the patients who do not prefer surgical treatment including myringoplasty.

　It is conceivable that continuous supply of growth factors brought about by daily eardrops promotes closure of TM perforations.
