 A novel definitive diagnostic test for perilymphatic fistula(PLF)has been established using Cochlin-tomoprotein(CTP), a specific protein found in the perilymph, as a biochemical marker. Based on the CTP detection test, leakage from the fistula can be evaluated objectively. Several study groups have reported cases of PLF with merely mild symptoms, such as vertigo only.

 We herein report a case of a traumatic PLF caused by an earpick without sensorineural hearing loss or vertigo, in which the final diagnosis was made with the CTP detection test.
