キチン膜を用いた鼓膜穿孔閉鎖術の検討

A clinical study of eardrum perforation closure treatment with chitin membrane

はじめに

　鼓膜穿孔閉鎖術は，耳周囲に皮膚切開を必要とする鼓室・鼓膜形成術と比べ，慢性鼓膜穿孔に対する低侵襲で安価な治療である。その有用性は高く，近年では自己血清^1-3），濃厚血小板血漿^4），basic fibroblast growth factor（bFGF）製剤^5-7）を使用する方法などのさまざまな報告が散見されるが，治療の特殊性から市中病院での導入はやや困難と思われる。これらに対してキチン膜はきわめて安価であるが，これまでの報告では，これを単独で用いた方法は鼓膜閉鎖率が低いと認識されている^2,8,9）。本方法を特殊な製剤を用いずに行ったため，その結果について報告する。

　A simple treatment for eardrum perforation with chitin membrane was studied in the outpatient clinic. Eardrum perforations were closed in 67.5％（27 ears）of all 40 ears. The average number of trials was 3.5 times. Closure rate was higher in the patients under 15 years old（87.5％, 14/16 ears）, and the group except chronic otitis media（84.0％, 21/25）. Generally, eardrum perforation closure treatments in the outpatient clinic were recognized to be not so successful, but can be effective for at least these patients.
