ABRでⅢ波以降が消失しながら，良好な聴力像を呈したPelizaeus-Merzbacher病の2例

Two infants of Pelizaeus-Merzbacher disease with abnormal ABR configurations and fairly preserved hearing ability

I．はじめに

　Pelizaeus-Merzbacher病（以下，PM病）は，Pelizaeus^1）やMerzbacher^2）によって報告された遺伝性疾患で，中枢神経系の髄しょう形成不全をきたす症候群である。現在では，主に伴性劣性遺伝により，Xq22染色体のPLP遺伝子の変異や欠損により生じるといわれている^3）。新生児期から白質の髄しょう化がほとんど進行しない予後不良な重篤な疾患で，神経耳科的な検索はほとんどなされていない。幼少時期から神経発達遅滞や運動神経障害を伴うため，純音聴力検査，語音聴力検査などの聴覚機能の評価を正確に行うことができなくauditory brainstem response（以下，ABR）での検索が行われているにすぎない。加我ら^4）によるPM病の症例では，ABRでⅠ，Ⅱ波のみを示し，Ⅲ波以降が消失するとしている。一般にABRでⅠ，Ⅱ波のみを示す場合，聴神経腫瘍，小脳橋角部腫瘍，橋部の腫瘍や出血，変性疾患，さらには意識障害の脳死に近い時期などに認められる^5）。このようなⅠ，Ⅱ波のみを示す後迷路性難聴の症例では，純音聴力閾値に比べて言語理解が乏しいのが特徴である。

　今回，われわれは，中枢神経の髄しょう化障害で白質変性症であるPM病の2例に神経耳科学的検査を施行し，PM病の聴力機能を評価した。PM病では，ABRの結果だけでは聴覚機能を正確に示すことができないことが示唆された。原疾患の病態と聴力像の因果関係について，考察したので報告する。

We report the audiological and radiological findings including auditory brainstem responses（ABR）in two isolated Japanese boys with Pelizaeus-Merzbacher disease. Although both patients show wave Ⅰ only or wavesⅠ and Ⅱ in ABR,their hearing ability is preserved by conditioned orientation response audiometry（COR）. T2 weighted MRI revealed high intensity of white matter and low intensity of gray matter in the cerebrum and spinal cord；These findings indicate the demyelinated area of the brain. The reason why discrepancy between the ABR and audiological findings is not clear.
