I．はじめに

　近年，副鼻腔真菌症の報告例は増加傾向にある。その原因としてステロイドや抗菌薬の頻用，基礎疾患による免疫力の低下などが指摘されている^1,2）。その菌種としてはアスペルギルスが最も多く，ほとんどの症例では鼻腔・副鼻腔に限局する寄生型に分類されるが^3），稀に破壊性に進展する侵襲型の経過をたどることがある。

　今回われわれは，副鼻腔から頭蓋底に広範囲に浸潤し多発脳神経麻痺をきたし，（1-3）-β-D-glucan測定が診断に有用であった侵襲型アスペルギルス症例を経験したので報告する。

Mycosis in the sinonasal tract has recently been increasing,and it is recognized in the immunocompromised patients who those with chronic debilitating diseases such as diabetes mellitus. Aspergillus is the most common fungal pathogen in paranasal sinus disease,and Aspergillus infections are classified as non-invasive and invasive.

　We report a case of invasive aspergillosis presented with multiple cranial nerve palsy. This case illustrated the difficulty of establishing the diagnosis of invasive aspergillosis and differentiating from other infections or neoplastic entities. Serum（1-3）-β-D-glucan measurement was useful procedure for diagnosis of invasive deep mycosis,unexceptionally for invasive aspergillosis originated from the paranaasl sinus.
