急激に視力障害をきたした副鼻腔炎の1症例

A case of sudden and severe visual disturbance due to sinusitis

Ⅰ．はじめに

　副鼻腔炎の重篤な合併症として，周囲組織への炎症波及による眼窩内合併症がある。眼球運動障害や視力障害をきたした症例では，診断や治療が遅れると後遺症を残すことがあり，迅速な診断と適切な治療が望まれる^1）。

　今回，外傷以外に明らかな副鼻腔炎の既往がなく，症状出現から1日で視力障害をきたした篩骨洞，前頭洞炎の症例を経験した。本症例の発症に関して，若干の文献的考察を加えて報告する。

□Abstract

　A 20-year-old man presented with left orbital pain. On head CT scan,no abnormality was detected in the cerebrum. When he returned to his home from hospital,sudden and severe orbital disturbance appeared. Orbital MRI revealed a subperiosteal abscess. Immediately,external frontal antrotomy was performed,but his visual disturbance was remained. A half year before,he received a severe injury around the left orbit. It was suspected that sinusitis had been induced by this orbital injury.
