Abstract 文献概要

　ぶどう膜炎とともに発症し色素性傍静脈網脈絡膜萎縮が進行し，inflammatory pigmented　paravenous retinochoroidal atrophy （IPPRA）として報告した症例を，3年間経過観察し長期経過を検討した。眼底検査では，網膜静脈に沿う特微的な約1乳頭径の網膜色素上皮の変性萎縮が連続し進展していた。両眼とも変性部の色素沈着が増強，後極部の網膜色素上皮が粗糲化し，左黄斑部には色素上皮の増殖と考えられる所見があった。螢光眼底撮影では，ほぼすべての静脈に沿い約1乳頭径の幅で，網膜色素上皮の萎縮に起因すると考えられる過螢光像が示された。左眼では乳頭周囲および黄斑部を中心とした後極部に過螢光と低螢光がモザイク状に交錯する像が示された。3年の経過で視力，視野およびERGの検査所見はすべて悪化した。

A 50-year-old male presented with blurring ofvision in both eyes since 6 months before. Visualacuity was 0.8 and intraocular pressure was 16mmHg in either eye. He showed signs of iritis andvisual opacity in both eyes. Funduscopy showedparavenous retinochoroidal atrophy in both eyes.Systemic and topical corticosteroid failed to pre-vent further aggravation of iridocyclitis, vitreousopacity, complicated cataract and extension ofatrophy along major retinal veins towards theperiphery. Three years later, the visual acuitydetriorated to 0.02 in both eyes with constrictedvisual field. The findings illustrate that inflamma-tory pigmented paravenous retinochoroidal atro-phy may be progressive with poor visual outcome.
