　深部表層角膜移植術を行った16例18眼の成績を検討した。原疾患は，格子状角膜変性症7眼，角膜白斑6眼，その他であった。全例に新鮮角膜を用いた。術後6〜82か月，平均22か月の経過観察を行った。移植片の透明治癒が14眼（77.8％）で得られ，4眼には再手術を行った。全例で視力が向上し，0.3以上の視力が14眼（78％）で得られた。術後6か月での乱視は2.4±1.3Dであり，本術式では良好な角膜形状が得られやすいと考えられた。深部表層角膜移植術は重篤な合併症が少なく，移植片の透明性や形状が良好であり，角膜内皮が比較的健常な角膜疾患に対して有効な術式であると結論される。

We reviewed 18 eyes of 16 patients who underwent deep lamellar keratoplasty. The causative lesion comprised lattice corneal degeneration 7 eyes, corneal leukoma 6 eyes, and others. Fresh donor cornea was used throughtout. The postoperative follow-up ranged from 6 to 82 months, average 22 months. The graft remained transparent in 14 eyes (78%). The remaining 4 eyes received a second surgery. The visual acuity improved in all the eyes. Visual acuity of 0.3 or over was obtained in 14 eyes (78%). The corneal astigmatism averaged 2.4±1.3 diopters at 6 months after surgery. The findings show that deep lamellar keratoplasty is effective for corneal lesions with relatively intact corneal endothelium because of few severe complications, lesser corneal deformity, and lesser incidence of graft opacification.
