眼内灌流液によるコアリングの経験

An instance of coring in the intraocular irrigating solution

　白内障手術の際に，灌流液BSS PLUS^TM中に浮遊している黒色異物を手術室の看護婦が発見した。製造元の検査で，灌流瓶のゴム栓の一部が針を穿刺した際に削りとられて液中に落下したコアリングであると確認された。当日に行われたすべての手術患者に，眼内異物や眼感染症は起こっていない。製造元によると，コアリングの発生率は約0.01％である。その防止策として，針を刺入部の中央に垂直に刺してねじらないこと，複数回の刺入を避けることなどが挙げられる。また，万一これが発生したときのために，フィルタの使用，灌流瓶内の異物の有無をチェックすることなどの対策が望まれる。

During cataract surgery, a nurse in the surgical theater detected a black particle in the bottle of irrigating solution BSS PLUS^TM. Investigations by the manufacturer showd the particle to be an instance of coring, or a piece of rubber stopper of the bottle which fell of during insertion of a needle. The incidence of coring is about 0.01% according to the report of the munufacturer. In order to avoid coring, we advocate inserting the needle to the center of the rubber stopper without screwing motion or reinsertion. We also advocate use of an adequate filter and close observation of the irrigating solution in the bottle.
