多発性の遷延化した中心性漿液性脈絡網膜症に対する光線力学療法

Photodynamic therapy for prolonged central serous chorioretinopathy with multiple points of dye leakage

要約　目的：低複色素漏出点があり，遷延化した中心性漿液性脈絡網膜症に光線力学療法を行った2症例の報告。症例：1例は63歳男性で，2年前に1か所の漏出点に光凝固を受けて治癒し，再発で受診した。矯正視力は0.3で，2個の新しい漏出点があった。他の1例は59歳男性で，3年前から変視症があった。矯正視力は0.1で，鮮明な漏出点とびまん性が各1個と2か所の網膜色素上皮剝離があった。両症例に照射エネルギー量を半分にした光線力学療法を行った。漿液性網膜剝離は消失し，視力はそれぞれ0.6と0.5に改善した。結論：照射エネルギー量を半分にした光線力学療法は，再発性または遷延化した中心性漿液性脈絡網膜症に奏効することがある。

Abstract.　Purpose：To report two cases of prolonged central serous chorioretinopathy（CSC）with multiple points of dye leakage treated by reduced fluence photodynamic therapy. Cases：Both were males aged 63 and 59 years respectively. One case had CSC 2 years before that cured after photocoagulation. He now presented recurrence in the same eye with 2 new points of dye leakage. He had corrected visual acuity of 0.3. The other had noted metamorphopsia since 3 years before. The affected eye showed one discrete and diffuse points of dye leakage with 2 areas of retinal pigment epithelial detachment. Both cases received photodynamic therapy at one half of energy level. CSC disappeared in both cases. Visual acuity improved to 0.6 and 0.5 respectively. Conclusion：These cases illustrate that reduced fluence photodynamic therapy may be effective for recurred or prolonged CSC with multiple points of dye leakage.
