足柄上医師会は,10年間にわたり神奈川県足柄上地区(人口11万人)の糖尿病患者を登録管理してきた。患者数は人口の2.1%,40歳以上人口の42%,糖尿病網膜症(以下,網膜症)の合併率は年平均10.7±0.4%であった。網膜症合併患者の年平均35.8%が安藤眼科医院を受診しており,病型(福田分類)は,A群73.3±4.3%,B群26.7±4.3%であった。1991年までの網膜症眼はそれ以降に比して低視力(0.4以下)が有意に多かった。1997年に視力0.4以下だった網膜症眼は243眼中75眼(30.9%)であり1うち48眼(64.0%)は黄斑病変による視力低下であった。243眼の網膜症(1997年)に対して,網膜光凝固術が180眼(74.1%)に,硝子体手術が9眼(3.7%)に行われていた。
The Ashigarakami Medical Association has been recording and managing diabetic patients in Ashigarakami area since 10 years ago. This area is located near Metropolitan Tokyo and has a population of about 110,000. A total of 2.1% of the population, or 4.2% of persons over 40 years of age, was found to have diabetes so far. The annual incidence of retinopathy averaged 10.7 ± 0.4% of diabetic patients. One eye clinic in this area was visited by an annual average of 35.8% of patients with retinopathy. They comprised 73.3 ± 4.3 % of type A and 26.7 ± 4.3% of type B retinopathy after the classification by Fukuda. Cases seen before 1991 had a significantly higher rate of visual acuity of 0.4 or less. Out of 243 eyes with retinopathy seen during 1997, 75 eyes (30.9%) had visual acuity of 0.4 or less, including 48 eyes (64.0%) due to maculopathy. Out of these 243 eyes, photocoagula-tion was performed in 180 eyes (74.1%) and vitreous surgery in 9 eyes (3.7%).
