The Ashigarakami Medical Association has been recording and managing diabetic patients in Ashigarakami area since 10 years ago. This area is located near Metropolitan Tokyo and has a population of about 110,000. A total of 2.1% of the population, or 4.2% of persons over 40 years of age, was found to have diabetes so far. The annual incidence of retinopathy averaged 10.7 ± 0.4% of diabetic patients. One eye clinic in this area was visited by an annual average of 35.8% of patients with retinopathy. They comprised 73.3 ± 4.3 % of type A and 26.7 ± 4.3% of type B retinopathy after the classification by Fukuda. Cases seen before 1991 had a significantly higher rate of visual acuity of 0.4 or less. Out of 243 eyes with retinopathy seen during 1997, 75 eyes (30.9%) had visual acuity of 0.4 or less, including 48 eyes (64.0%) due to maculopathy. Out of these 243 eyes, photocoagula-tion was performed in 180 eyes (74.1%) and vitreous surgery in 9 eyes (3.7%).
