単純ヘルペス1型結膜炎の臨床像

Clinical features of acute conjunctivitis due to herpes simplex virus type 1

（B5-2-9）　急性濾胞性結膜炎478例のうち単純ヘルペス1型（HSV−1）が分離された23例（4.8％）を対象に，臨床的，ウイルス学的検討を加えた。10歳以下は13％であったが，21歳以上が65％を占め，成人の割合が多かった。片眼性が87％と多く，治癒までは平均8.3日と短かったが，臨床所見から初診時にアデノウイルス結膜炎との鑑別は困難であった。また，結膜擦過材料に対する直接螢光抗体法（Micro Trak HerpesR）では全例が陰性であり，角膜株と結膜株との間に生物学的相違がある可能性が示された。成人における急性結膜炎の病因には，HSVをも考慮することが必要である。

We reviewed a series of 478 cases of corneal herpetic lesions or acute follicular conjunctivitis seen in an eye clinic in Hokkaido area during 12 years through 1994. Herpes simplex virus type 1 (HSV-1) was isolated in 23 cases (4.8%). The latter series comprised 3 children under 10 years of age (13%) and 15 adults aged 20 years or over (65%) . Only one eye was involved in 87% of the cases. It took an average of 8.3 days before clinical cure. It was difficult to differentiate cases due to HSV-1 from those of adenoviral conjunctivitis based on clinical features during the early stage of the disease. All cases were negative when examined by direct fluorescence antibody method, showing the possibility that biological difference may be present between strains obtained from the cornea and conjunctiva. The present findings illustrate that HSV may be involved in the etiology of acute conjunctivitis in adults.
