　55歳の男性が両眼の視力低下で受診した。2か月前に歩行中の交通事故で頭部に鈍性外傷を負い，脳内血腫と診断されていた。矯正視力は両眼0.5で，典型的な両鼻側半盲があった。右眼に乳頭の蒼白化と両眼に緑内障性の陥凹拡大があった。頭部MRI検査で，右前頭葉と側頭葉におよぶ血腫があり，視交叉が左側に偏位し，下垂体漏斗が左側に傾斜していた。両鼻側半盲は，視交叉への血腫による圧迫に加え，左側への視交叉の偏位により，隣接している内頸動脈がこれを左側から圧迫した結果であると推測された。

A 55-year-old male presented with blurring of vision in both eyes. He had suffered from cerebral hematoma following traffic accident two months before. The corrected visual acuity was 20/40 in either eye. Perimetry showed typical binasal hemianopsia. The right eye showed pale optic disc. Glaucomatous disc cupping was present in both eyes. Magnetic resonance imaging (MRI) showed hematoma in the right frontal and temporal lobes with deviation of the optic chiasm and infundibulum to the left. The binasal hemianopsia appeared to be the consequence of pressure to the optic chiasm by the hematoma and of counter pressure by the left carotid artery.
