We treated 38 glaucomatous eyes by simultaneous intraocular lens implantation, trabeculotomy and sinusotomy. The series comprised primary open angle glaucoma 25 eyes and capsular glaucoma 13 eyes. Another series of 37 eyes underwent trabeculotomy with sinusotomy alone. These eyes comprised primary open angle glaucoma 29 eyes and capsular glaucoma 8 eyes. The postoperative intarocular pressure (IOP) was significantly lower in the former group receiving triple procedure at 6 months after surgery. When the successful IOP level was defined as 14 mmHg and 20 mmHg respectively, there was no difference in the rate of success thereafter until at least 30 months after surgery. Both groups needed reduced medications for glaucoma after surgery. The findings show that cataract surgery with intraocular lens implantation does not show significant pressure-lowering effects after simultaneous trabeculotomy with sinusotomy.
