　上眼瞼に発生した外毛根鞘癌を経験した。症例は72歳の女性で，左上眼瞼に発生した腫瘍に対し，腫瘍を切除し，眼瞼を形成した。病理組織学的検査で，腫瘍は胞体に豊富なグリコーゲン顆粒を持つ明調細胞から成り，一部には毛包様の角化，小葉構造辺縁部での柵状配列などがみられた。また，腫瘍細胞は細胞異型，核異型が強く，数多くの細胞分裂像がみられた。以上のことから，外毛根鞘癌と診断した。

A 72-year-old female presented with gradually enlarging tumor in the left upper eyelid of two years' duration. Histopathological studies of the resected lesion showed tumor cells with clear cytoplasm and atypical nucleus proliferating in multilobular configuration both in the dermis and epidermis. The cytoplasm contained numerous granules. PAS staining with or without diastase digestion showed these granules to be glycogen. The cancer cells were clearly differentiated from hair follicles. Mitotic figures were frequent. These findings led to the diagnosis of the tumor as trichilemmal carcinoma.
