上眼瞼原発の外毛根鞘癌再発例に対する外科的治療

Surgical Treatment of Recurrent Trichilemmal Carcinoma of the Upper Eyelid

I．はじめに

　外毛根鞘癌（trichilemmal carcinoma：以下，TLCと略す）は毛包の外毛根鞘由来と考えられる皮膚悪性腫瘍で，1976年にHeadington^1）により提唱された。本邦においては森岡ら^2）によってmalignant trichilemmomaとして初めて記載された。本腫瘍は高齢者の日光曝露部位に好発するとされており，頭頸部は好発部位の1つであるが，近年本邦においては皮膚科領域を中心に年に十数例が報告されている^3～5）。しかし，眼瞼における本腫瘍の報告例は極めて少なく，国内外の文献を合わせても数例みられるのみである^6～10）。

　今回われわれは，上眼瞼原発の外毛根鞘癌が眼窩内に浸潤した再発例の外科的治療を経験したので報告する。

　A 34-year-old woman had one month history of a swelling of her left upper eyelid. She received full dose irradiation for trichilemmal carcinoma of the upper eyelid and three times of endovascular treatment but the tumor recurred. The tumor was located at the lateral half of her left upper and lower eyelids and invaded to the soft tissue in the orbit towards the orbital apex. The tumor was resected with eyelids and orbital contents. She had no evidence of local recurrence and metastases 29months after the surgery.
