原発開放隅角緑内障におけるウノプロストン点眼液の変更薬剤としての有用性

Efficacy of unoprostone ophthalmic solution as replacement therapy for primary open angle glaucoma

（26D-17）　原発開放隅角緑囚障患者のうち，比較的眼圧のコントロールは良好なものの，従釆受用していた緑内障治療薬で眼充血，眼痛などの副作用が発現している症例や呼吸器系疾患などの全身疾患を有する症例に対してウノプロストン点眼液への変更を試みた。ウノプロストン点眼液へ変更した対象40例の，変更前の薬剤はジビベプリン点眼薬25例，ピロカルピン点眼薬10例，β遮断剤点眼薬4例，炭酸脱水酵素阻害剤内服薬1例である。変更前眼圧値（mean±SD）18.8±3.9mmHgと比較して，変更1か月後の眼圧下降度は1.5±2.5mmHg（p＝0.0006）を示し，変更時の条件でウノプロストンの継続投与が可能であった症例は79.5％であり，本剤を変更薬剤として使用することは充分有用であると思われた。

We evaluated the value of isopropyl unoprostone ophthalmic solution asreplacement therapy in 40 patients with primary open angle glaucoma. They had shown adverse effects of prior medication including redness, irritation or systemic complications with respiratory or cardiovascular diseases. They had been using, as prior medication, topical divivefrine 25 cases, topical pilocarpine 10 cases, ,3 -blockers 4 cases and systemic acetozolamide 1 case. One month after replacement by unoprostone, the intraocular pressure lowered by 1.5 ± 2.5 mmHg from the initial value of 18.8± 3.9 mmHg (p =0.0006) . Superficial punctate keratopathy developed in 3 cases. Iris pigmentation or systemic adverse effect was consistently absent. The findings show the value of isopropyl unoprostone ophthalmic solution as replacement therapy for primary open angle glaucoma.
