　後房レンズの毛様溝縫着術はさまざまな術式が報告されているが，術後無水晶体眼で角膜内皮細胞1000／mm²以上の50例53眼に対して，Lewisらの原法に準じ比較的容易な方法を試みた。術中毛様溝通糸時の出血を2例，術後脈絡膜剥離を2例認めたが，両者とも軽度で術後短期間で消失し術後視力に影響はなかった。本術式では硝子体切除を術終了前にレンズ上で必要最小限に行っただけであるが，網膜剥離など重篤な合併症は認めていない。また，臨床上問題となるほどのレンズの偏位や傾斜も認めていない。本術式による眼内レンズ毛様溝縫着術は，特殊な器械を必要とせず比較的容易で安全に行うことができ，術後経過も現在のところ良好である。

We performed secondary posterior chamber lens implantation by transscleral sulcus fixation with suture in 53 eyes. Anterior vitrectomy was performed at the end of surgery: Postoperative visual acuity was generally favorable during the follow-up of one year or more. Complications developed in 4 eyes: choroidal detachment in 2 eyes and intraocular hemorrhage from the ciliary body due to the transscleral suture in 2 eyes. These complications were mild and did not interfere with the postoperative visual acuity. There was no instance of retinal detachment, anterior proliferative vitreoretinopathy or marked decentra-tion of the intraocular lens. This method is advocated because of its safety, easiness and favorable outcome.
