ベーチェット病とVogt—小柳—原田病における可溶性接着分子および可溶性細胞表面マーカーの動態

Dynamics of intercellular adhesion molecules and interleukin-2 receptor in Behçet and Vogt-Koyanagi-Harada diseases

　ベーチェット病（BD）20例とVogt—小柳—原田病（VKH）20例の血清中の可溶性ICAM−1（sICAM−1），可溶性CD4（sCD4），可溶性CD8（sCD8）および可溶性インターロイキン2レセプター（sIL−2R）を測定し、疾患活動性を評価する指標となり得るかどうかを検討した。slCAM−1は，BDおよびVKHの急性期に，対照より有意に上昇し，BDでは寛解期に眼発作期より有意に上昇していた。sCD4とslL−2Rは，活動性BDおよびVKHの急性期に対照より有意に上昇し，疾患活動性に相関した変動もみられた。sCD8は活動性BDとVKHの急性期および回復期で対照より有意に上昇していた。可溶性マーカー相互ではsIL−2RとsCD4が，BDおよびVKHの両者で有意な相関を示した。以上からこれらの可溶性マーカーはBDおよびVKHの活動性を示す指標になり得ると考えられる。

We measured the serum levels of soluble form of ICAM-1, CD4, CD8 and interleukin-2 receptor (IL-2R) in 20 patients with active Behçet disease and 20 with Vogt-Koyanagi-Harada disease (VKH). The level of ICAM-1 was significantly higher in both diseases during the active or acute stage as compared with 20 normal persons. In Behçet disease, the level of ICAM-1 was significantly elevated in convalescent stage than in acute stage. The levels of CD4 and I1-2R were significantly higher in Behçet disease in both stages and in VKH in the acute stage than in controls. In both diseases, IL-2R levels were significantly less during the convalescent stage than in acute stage. In both diseases, the levels of CD8 were significantly increased than in controls. The CD4 and IL-2R levels were positively correlated in both disease during the acute stage. The findings show that these soluble markers may serve as potentially useful parameters in monitoring the activity of both diseases.
