　過去10年間に岡山大学医学部附属病院眼科外来を受診したむちうち症7例を対象として，輻輳不全と調節障害の有無，その経過および治療予後について検討した。7例全例に調節力の低下がみられた。輻輳近点を測定できた6例のうち，3例では正常，1例では延長し，2例では輻輳不能であった。輻輳近点の延長と調節力の低下が同時にみられたのは3例であった。調節障害に対しては凸レンズ付加の近用眼鏡を，輻輳不全には基底内方プリズムを付加した近用眼鏡を処方した。経過観察中に自覚症状は改善したが，輻輳機能や調節力の改善はみられなかった。眼科的不定愁訴のあるむちうち症では，調節障害や輻輳不全を念頭に診療にあたる必要がある。

We reviewed 7 patients with insufficiency of convergence and accommodation secondary to blunt trauma to the head and neck, or whiplash injury, during the past 10-year period. Accommodation was reduced in all the 7 patients. convergence was normal in 3, weakened in 3 and absent in 2. Subjective symptoms improved in all cases by wearing convex lenses for near and/or base-in prisms. The state of insufficiency of convergence and accommodation persisted throughout. Convergence and accommodation insufficiency has to be suspected when patients present with ophthalmological complaints after whiplash injuries.
