放射線照射が著効した転移性虹彩腫瘍の1例

Radiation was effective in a case of metastatic carcinoma of the iris

　肺扁平上皮癌患者の虹彩にみられた転移性腫瘤に対して放射線照射を行った症例を経験した。症例は72歳女性で，左眼の流涙，充血を訴え，当科を紹介された。初診時，左眼6〜8時の虹彩面上に2個の灰白色の腫瘤がみられた。前房穿刺により扁平上皮癌細胞が確認された。眼底には異常所見をみなかった。虹彩腫瘤が徐々に増大してきたため，放射線療法（2Gy／日，週5回）を開始したところ1週間後に腫瘤は半分に縮小し，総量30Gyの照射でほぼ消失したが，脳転移とともに再発し，3か月後に死亡した。本症例を含めて1960年以降わが国で報告された39例の虹彩転移腫瘍の予後は悪く，患者の生活の質を考えた治療法が選択されるべきである。

A 72-year-old female presented with lacrimation and hyperemia in the left eye. She had been diagnosed as squamous cell carcinoma of lung 3 months before. Two grayish-white tumors were present on the iris in the affected eye. Squamous carcinoma cells were detected in the aspirated aqueous. No fundus lesions were present. Radiation totalling 30 Gy led to complete regression of the iris tumor in 3 weeks. The patient died 3 months later from metastatis to the brain. A review of 38 reported cases of iris metastatic tumor, including the present one, showed a mean life expectancy of 4.1 months. A proper therapeutic approach will have to be chosen on the basis of quality of life in these patients.
