免疫組織化学染色にて診断した結膜悪性リンパ腫の2例

Immunohistochemical diagnosis in two cases of primary conjunctival malignant lymphoma

　結膜原発の悪性リンパ腫を2例経験した。2例とも片眼性であり，結膜悪性リンパ腫に特徴的な境界鮮明なサーモンピンク色調の腫瘍を形成し，1例は他に掻痒感，眼痛等を訴え，もう1例は充血，異和感，流涙を主訴とした。結膜原発の悪性リンパ腫は非ホジキンリンパ腫B細胞型びまん性小細胞型，中細胞型が多いという従来の報告に一致して，1例は小細胞型，1例は中細胞型であった。これらのタイプは低〜中等度悪性で，2例とも全身転移はなく結膜に限局しており，従来の放射線療法，化学療法の効果と副作用を考慮し，患者の同意を得た上で未治療のまま経過観察しているが，現在のところ腫瘍の増大はない。リンパ系腫瘍の場合，生検組織の免疫組織化学染色の結果に基づいて，治療法を選択することが重要である。

Primary malignant lymphoma of bulbar and palpebral conjunctiva was diagnosed in two cases. Both cases were unilateral and manifested salmon-pink tumor with well-defined margins. Histologically, it was diagnosed as diffuse malignant lymphoma of small-sized cell type in one case and of medium-sized cell type in the other. Immunohistochemical staining showed the tumor to be of B cell type in both cases. Both cases were followed up without treatment under informed consent, since the lymphoma showed low-grade malignancy and was localized to the conjunctiva. Immunohistochemical staining of biopsied tissue gave guidelines for selecting the therapeutic modalities.
