眼内異物によってひき起こされた続発緑内障の1例

A case of secondary glaucoma due to intraocular ferrous foreign body

　2年間放置されていた眼内鉄片異物によりひき起こされた続発緑内障の1例を報告した。

　症例は39歳，男性。初診時視力は右1.5，左H．B．。眼圧は右16mmHg，左72mmHg。異物は左眼網膜に接して存在し，被膜に覆われていた。左隅角は開放隅角であった。Electroretinogram（ERG）のa波，b波，律動様小波（OP）は減弱していた。左眼硝子体切除術，硝子体マグネットを用いた眼内異物摘出，およびトラベクレクトミーを施行した。摘出した異物は重量0.5mg，微量分析によりマルテンサイト系ステンレスと推察された。

　本症ERGのa波，b波，OPは減弱しているものの消失型ではなかったのは，①異物が腐食されにくいステンレスであったこと，②異物が被膜に覆われていたことによると思われた。

A 39-year-old male had suffered from penetrat-ing injury by a ferrous foreign body in his left eye 5 years before. His visual acuity was reduced to hand movement with intraocular pressure of 72 mmHg. The foreign body was encapsulated and was located in the inferior nasal periphery of the vitreous. Gonioscopic findings were normal.Electroretinogram showed reduced but not extin-guished oscillatory potential, a- and b waves. The foreign body was removed by vitrectomy and intraocular magnet. It weighed 0.5 mg and proved to be of stainless steel, martensite group, from microchemical analysis. It appeared that the electroretinogram was still recordable because the foreign body was encapsulated and was of non-corrosive stainless steel.
