向精神薬によると思われる緑内障悪化症例

Exacerbation of angle-closure glaucoma during psychotropic medication

　76歳女性で，向精神薬の服用によると思われる緑内障悪化症例を報告した。両眼とも狭隅角を呈していたが来院が途切れ，その後躁欝病のため精神科病院への入院期間があった。入院中緑内障発作が生じ，近医で加療を受けるも，当科再診時には右眼は既に失明に至っていた。その間抗コリン作用を有する向精神薬の服用が認められ，薬剤による散瞳効果および毛様筋への影響による房水流出障害により眼圧上昇をきたしたものと考えられた。精神科疾患患者においては訴えが的確に把握され難く，また緑内障を悪化させる薬剤の投与が予想されるため，緑内障とりわけ狭隅角を呈する場合は，レーザー虹彩切開を含めより積極的な治療が必要と思われた。

A 76-year-old female was under topical medical treatment for chronic angle-closure glaucoma. The medication was discontinued when she was hospitalized due to manic depressive episodes. An acute glaucomatous attack developed during hospi-talization resulting in loss of the affected right eye. She was treated with systemic maprotiline hydroch-loride, an anticholinergic agent, during hospitaliza-tion. Use of psychotropic agent seemed to have induced mydriasis and relaxation of ciliary mus-cles, triggering a glaucomatous attack. Prophylac-tic laser iridotomy is advocated for persons with narrow angle prior to treatment with psychotropic drug with anticholinergic properties.
