32年後に緑内障を発症した眼内鉄片異物の1例

A case of glaucoma secondary to intraocular iron body 32 years after injury

　32年後に緑内障を発症した，眼内鉄片異物の症例を経験した。症例は51歳男性で，32年前の眼内鉄片異物により続発緑内障を発症し，眼圧コントロールのために，1991年6目に線維柱帯切除術を施行した。その後，眼内異物除去と視力改善目的に，1993年5月，超音波水晶体乳化吸引術，眼内レンズ挿入術，硝子体切除術，輪状締結術，異物除去術を施行した。眼内異物による続発緑内障は予後不良といわれているが，線維柱帯切除術後約5年を経て，眼圧は無投薬で14mmHg，矯正視力1.2と眼圧コントロールおよび視機能の経過は良好である。

A 51-year-old male was referred to us for presumed open angle glaucoma in the right eye of recent onset. He had retained an asymptomatic intraocular foreign body for 32 years. Trabeculectomy induced normaliza-tion of intraocular pressure. He underwent, 2 years later, extraction of the foreign body by vitrectomy with cataract extraction and intraocular lens implantation. He is doing well with full visual acuity and normal intraocular pressure without medication after 5 years of trabeculectomy.
