　右眼上眼瞼腫脹を主訴とした65歳女性の硬膜頸動脈—海綿静脈洞痩の症例に対して脳外科医指導のもとにMatas法による治療を開始した。その後，主訴は改善したが，突然右眼視力低下が出現し，網膜中心静脈閉塞症をきたした。Matas法による治療を中止後，一時的に視力が改善したが，その後網膜に再出血をきたし，視力低下が進行した。

　Matas法による治療は硬膜頚動脈—海綿静脈洞痩に有効であるが，本症のように網膜循環障害を惹起する危険があるのではないかと推測する。

A 65-year-old female presented with upper eyelid swelling, proptosis and dilated vessels in the bulbar conjunctiva in her right eye. Detection of a bruit and carotid angiography led to the diagnosis of right dural carotid-cavernous fistula. Proptosis improved after treatment with Matas technique. Sudden decrease in right visual acuity occurred on day 25 of treatment. We detected a few vitreor-etinal hemorrhages in the right posterior fundus. Central retinal vein occlusion developed 2 months later with further decrease in vision in the affected eye. After discontinuation of Matas technique, the vision improved again. Treatment with Matas tech-nique was suspected as the cause of interference with the retinal circulations.
