I．はじめに

　頸動脈海綿静脈洞瘻（以下CCFと略す）は従来外傷性と特発性に分類され^4），種々の治療法が試みられてはきたが^{1,2,4,7,13,16,21）}，いまだ確実に本症を治癒せしめる治療に乏しく，特に特発性CCFの場合には，その外科治療の成績は外傷性に比し悪いといわれている^14）．

　われわれは，当科開設後2年間に5例の特発性CCFを経験したが，そのなかで難治性の3症例に対して，開頭下にcopper needle（銅針）を海綿静脈洞内に挿入することによりelectrothrombosisを行い，比較的良好な結果を得た，ここにその概要とともに若干の考察を加え報告する．

Electrothrombosis by copper needle insertion wasapplied to three cases of spontaneous carotid cavernousfistula. These cases had been observed conserva-tively for about six months or more, but their symp-toms did not improve. On angiogram, the feedingarteries were branches of both the ipsilateral internaland the external carotid artery.

Frontotemporal craniotomy was performed andcopper needles were inserted transdurally intothe cavernous sinus. The closure of fistula wasverified by intra-operative angiography in all cases andtheir symptoms improved after operation.
