Abstract Purpose:To report a case of pediatric optic neuritis associated with acute disseminated encephalomyelitis(ADEM).

Case:A 5-year-old male child presented with fever, headache, lethargy and decreased vision since the previous day. He had received vaccination for measles and rubella one month before.

Findings and Clinical Course:Corrected visual acuity was 0.07 right and 0.04 left. Both eyes showed swollen optic disc. He was suspected of optic neuritis associated with ADEM. Magnetic resonance imaging(MRI)showed diffusely intense lesions in the white matter of the brain. Optic disc swelling subsided after 2 months of pulsed corticosteroid therapy. MRI showed new high-intense lesions in the brain. Serum tests were anti-myelin oligodendrocyte glycoprotein(MOG). Visual acuity improved spontaneously to 1.2 in either eye 2 months after onset. There has been no recurrence for 14 months until present.

Conclusion:The present case illustrates that ADEM may recur and may respond to systemic corticosteroid when associated with anti-MOB antibody. Measurement of anti-MOG antibody may be useful in the prognosis of patients with ADEM.
