Abstract Purpose:It was reported that benzalkonium chloride(BAC)causes damage of the cornea epithelium, reduction of tear, instability of tear film. Therefore, it is expected that after switching from latanoprost/timolol fixed combinations(LTFC)which contain BAC to carteolol/latanoprost fixed combination(CLFC)which does not contain BAC, damage of cornea epithelium would be reduced. Purpose of this study is to evaluate the effect on corneal surface and intraocular pressure(IOP)after switching from LTFC to CLFC.

Subject and Method:The study design is retrospective chart review. This study included primary open-angle glaucoma(POAG)patients who attended our hospital and developed superficial punctuate keratitis with tear film breakup time(BUT)of under 10 seconds during LTFC treatment for over 3 months. All patients were switched to CLFC. IOP, keratopathy(area-density score)and BUT were evaluated for 6 months after the switch. The primary endpoints were IOP, A(area)score, D(density)score, BUT at 6 months after switching.

Result:The data of 16 patients 31 eyes were evaluated. Statistically significant reduction of IOP(1.6 mmHg)was obtained 6 months after switching(p<0.001). Area score and density score improved significantly, the change were 1.4(A score)and 1.0(D score)after 6 months(p<0.001). BUT improved significantly, the change was 13.7 seconds after 6 months(p<0.001)

Conclusion:Switching LTFC to CLFC provides protective effects on ocular surface, maintaining intraocular pressure.
