ラタノプロスト・チモロールマレイン酸塩配合点眼からトラボプロスト・チモロールマレイン酸塩配合点眼への切り替えによる眼圧下降効果

Ocular hypotensive effect of switching from fixed combination of latanoprost-timolol to that of travoprost-timolol

要約　目的：ラタノプロスト・チモロール点眼液からトラボプロスト・チモロール点眼液に切り替えたときの眼圧下降効果の報告。対象と方法：ラタノプロスト・チモロール点眼液で治療中の広義の広隅角緑内障21例41眼を対象とした。男性5例，女性16例で，年齢は60～90歳（平均77歳）である。Washout期間なしにトラボプロスト・チモロール点眼に切り替え，3か月後までの眼圧と副作用を検索した。結果：眼圧は切り替え前の16.2±4.8mmHgから，1か月後14.2±4.0mmHg（p＜0.0001），3か月後14.0±3.4mmHg（p＜0.0001）に下降した。点眼時の自覚症状は，切り替え前は73％，切り替え後は22％にあった。角膜所見は35眼で不変，4眼で改善，2眼で悪化した。結論：ラタノプロスト・チモロール点眼液からトラボプロスト・チモロール点眼液に切り替えたとき，3か月までは平均眼圧が有意に下降し，副作用も減少した。

Abstract.　Purpose：To report the ocular hypotensive effect of switching from fixed combination of latanoprost-timolol to that of travoprost-timolol. Cases and Method：This study was made on 41 eyes of 21 patients who were receiving topical instillation of latanoprost-timolol for 3 months or longer. The medication was changed to travoprost-timolol without washout period. Intraocular pressure（IOP）and complication were evaluated for 3 months. Results：IOP averaged 16.2±4.8 mmHg before switching,14.2±4.0 mmHg（p＜0.0001）one month later,and 14.0±3.4 mmHg（p＜0.0001）3 months later. Adverse event was present in 73％ before and 22％ after switching. Keratopathy remained unchanged in 35 eyes,improved in 4 eyes,and worsened in 2 eyes. Conclusion：Switching from topical latanoprost-timolol to travoprost-timolol resulted in significant IOP decrease and reduced side effects.
