Abstract Purpose:To report a case who was treated by tram-track suture technique for recurrent pupil capture of sulcus-fixated intraocular lens(IOL).

Case:A 67-year-old male had received cataract surgery with IOL implantation in both eyes 10 years before. He noticed fluctuation in the left vision 3 months before and was treated by vitrectomy and insertion of sulcus-fixated IOL. He visited us for recent hyperemia and impaired vision in the left eye.

Findings and Clinical Course:Left visual acuity was 0.8 when corrected by sph −1.50D()cyl−2.25D 78°. The left eye showed hyperemia and pupillary captured IOL with intraocular pressure(IOP)of 26mmHg. Pupil capture of IOL recurred frequently after repositioning of IOL followed by miosis. Tram-track technique was then applied by introducing two 10-0 polypropylene monofilament sutures at 12 and 6 o'clock positions of the iris to block herniation of the IOL. The left eye showed improved vision, reduced astigmatism, and normalization of IOP. Area ratio of modulation transfer function improved from 14.3% before surgery to 21.2% after surgery. Optical coherence tomography showed two parallel threads anterior to the IOL. The patient has been doing well for 2 years until present.

Conclusion:Tram-track suture was useful in treating pupillary captured IOL. Optical coherence tomography allowed evaluation of dislocated IOL.
