硝子体出血をきたした老人性円盤状黄斑変性症の2例

Vitreous hemorrhage in senile disciform macular degeneration

　老人性円盤状黄斑変性症から硝子体出血を起した2症例3眼について述べた．　この中，第1例右眼は黄斑部のmottlingから，浮腫，漿液性ついで出血性網膜色素上皮剥離，硝子体出血，円盤状黄斑変性とGassの述べた進行様式を示した．第2例左眼も，途中からであるが，第1例に準じた進展を示した．

　第1例は両眼にウロキナーゼの硝子体内注入を行い，さらに右眼は水晶体摘出と前部硝子体切除を行ったが，結局は失明に終った．第2例はウロキナーゼの点滴静注が行われた．結果的には，観血的治療より，非観血的治療が好ましいことになるが，こうした出血を起す段階で，出血を防止する手段を講じることの必要性を痛感する．

　我々の外来で見られ，3ケ月以上follow upできた老人性円盤状黄斑変性症は，14例20眼で，硝子体出血を見たものは2例3眼である．

Massive vitreous hemorrhage occurred secondary to senile disciform macular degeneration in 3 eyes in 2 patients.

In the first case, a 72-year-old male, vitreous hemorrhage developed from the mottled area in the macula in the right eye during the course of obser-vation. In the fellow eye, massive vitreous hemor-rhage occurred consequent to hemorrhagic detach-ment of retinal pigment epithelium.

In the second case, a 71-year-old male, vitreous hemorrhage occurred secondary to apparent serous detachment of the retinal pigment epithelium asso-ciated with subretinal neovascularization. Numerous macular drusen were present in the fellow eye.

Systemic or intravitreal urokinase did not appear to favorably influence the clinical course.
