老人性円盤状黄斑変性症の病理組織学的検討

A histopathologic study of senile disciform macular degeneration

　症例は73歳女性で，老人性円盤状黄斑変性症の診断のもとに経過観察中硝子体出血，血管新生緑内障をきたし，持続する眼痛のため眼球を摘出した。その摘出眼球を病理組織学的に検討した。眼底後極部の網膜下および網膜色素上皮下には新生血管を含んだ増殖組織がみられ，一部の新生血管は脈絡膜血管との連続性が認められた。増殖組織内には多核巨細胞やマクロファージがみられた。網膜色素上皮下の新生血管壁が破綻して，網膜色素上皮下に出血をきたしていた。また網膜下出血や硝子体出血もみられた。ドルーゼンが後極部に散見され，一部のドルーゼンには新生血管が認められた。

We reported the histopathologic findings in one case of senile disciform macular degeneration, Fibrovascular proliferation was found in the sub-retinal pigment epithelial and subretinal spaces in the posterior pole. Some new vessels connected to the choroidal vessels were found in the fibrovas-cular tissue. Multinucleated giant cells and macro-phages infiltrated in the fibrovascular tissue. Sub-retinal pigment epithelial, subretinal and vitreous hemorrhages were noted. Drusen were scatteredly found in the posterior pole. Some drusen had new vessels.
