コーツ病光凝固後の長期観察例—蛍光眼底造影所見における二，三の新知見

Long term follow up of Coats' disease treated by photocoagulation.:New information obtained from fluorescein angiography

　6歳女子のコーツ病に対して光凝固を行い，その後7年余にわたり，螢光眼底造影により経過を観察し，以下の知見を得た。

（1）既存の血管瘤はprecapillary arterioleの細動脈からの分岐部にみられ，直接凝固されずとも，近傍の漏出著明な血管瘤の凝固により，小血管瘤は消失し，静脈側毛細血管の著明な異常透過性亢進も改善された。

（2）眼底周辺部における毛細血管網の螢光眼底所見の改善には，凝固後7年を要した。

（3）滲相性病変の沈静化した後，まったく健常と思われていた細動脈から新たに血管瘤の発生をみ，滲出液の漏出をきたすものもあった。この血管瘤形成に先だち，その部のprecapillary artcrioleから，しみ出し状出血を認めた。血管瘤の発生後，その近傍の静脈側の毛細血管の拡張をきたした。

　以上の所見から，コーツ病の進行には，血管瘤→滲出性病変→組織圧の亢進→毛細血管床の内圧亢進→precapillary arterioleの壁の脆弱性→血管瘤という悪循環が想定され，滲出性病変を増強させる因子として静脈側血管床の透過性亢進の関与も加えられた。しかし，本症にみられたような血管異常の発端としては，この悪循環のtriggerとして組織圧亢進をおこす微妙な滲出機転が考えられ，その根底に毛細血管壁の透過性の変化が想定された。

The clinical course over 7 years of Coats' disease in a healthy 6-year-old female treated by photo-coagulation is described. The fundus scopic and the fluorescein angiographic findings of the affected left eye indicated the followings.

1 ) The aneurysms at the precapillary arteriole disappeared without direct coagulation after the treatment of the adjacent larger aneurysms which had a remarkable fluorescein leakage. The dilated venous capillary bed with increased permeablility also normalized.
