オウム病が疑われたぶどう膜炎の1例

Presumed psittacosis in a case of uveitis

　オウム病抗体価の上昇を伴うぶどう膜炎の1症例を経験した。前房内炎症に加え，眼底所見では左黄斑部に白色出病変と漿液性網膜剥離を，両側眼底周辺部に小白点と毛細血管網の螢光漏出とを認めた。

　オウム病の眼所見に関する報告は少なく診断は困難であったが，咽頭痛などの上気道炎症状，インコ飼育歴，鑑別診断より本症例の原因としてオウム病が最も疑われた。

A 14-year-old girl presented with metamorphop-sia and paracentral scotoma in her left eye of 10 day's duration. We observed exudative lesion ac-companied by serous retinal detachment inferior to the left macula. Both eyes were affected by iridocy-clitis and small white patches in the retina. Fluores-cein angiography showed diffuse extravasation from capillaries. She showed elevated titer for psittacosis and inflammatory signs of upper respi-ratory tract. Parakeets had been bred in her home since childhood. We diagnosed her as presumed psittacosis. Traetment with systemic tetracycline and betamethasone led to clinical improvement in 4 weeks.
