乳頭上血管腫を認めたvon Hippel-Lindau病の1例

A case of von Hippel-Lindau disease with papillary hemangioma

　視神経乳頭上に発生した網膜血管腫を伴うvon Hippel-Lindau病の1例を報告した。螢光眼底造影で血管腫よりの螢光漏出が認められ，黄斑部に軽度の浸出斑がみられたが，視力良好のため経過観察された。CTスキャンでは異常所見は見いだせなかったが，MRIで小脳，延髄に腫瘍様所見が認められ，延髄については組織学的に血管芽腫と診断された。また膵，腎にもCTスキャンで嚢胞の合併がみられ，これらの所見より確定診断がなされた。

A 34-year-old woman was referred to us due to tumorous lesion in her left fundus detected by chance. We observed epipapillary hemangioma measuring 2 disc diameters across with full visual acuity. The right eye was intact. Magnetic reso-nance imaging showed space-occupying lesions inthe cerebellum and medula. Ensuing surgery showed the medullar lesion to be histopathological-ly confirmed hemangioblastoma. Computed tomo-graphy showed cystic lesions in the pancreas and kidney. She was diagnosed as von Hippel-Lindau disease.
