巨大裂孔網膜剥離に対する強膜側からのアプローチ

Transscleral surgery for retinal detachment with giant tears

　翻転網膜に可動性のある巨大裂孔網膜剥離7例7眼に対し，変形バックルによる強膜内陥術および冷凍凝固を施行した。シリコンスポンジを巨大裂孔後極側縁に沿ってなだらかに湾曲させバックリングすることにより，裂孔後極側縁網膜の皺襞をきたすことなく裂孔閉鎖が可能となった。術後全例で網膜が復位し，2例が再剥離し再手術を施行した。本法は術式が簡単で，しかも眼内気体注入などの眼内操作を行わないため，それによる合併症も起こらない。手術侵襲・復位率などの点からみて，優れた方法と考えられた。

We treated 7 eyes with retinal detachment due to giant tear and movable inverted posterior retinal flap. We applied modified scleral buckling and cryoretinopexy. Smoothly curved buckling along by the posterior edge of giant tear makes the tear close, because it prevents the retina from folding. Initial reattachment resulted in all 7 eyes, but 2 eye needed reoperation because of the recurrence of the retinal detachments. The above technique is easy and has few complication. It is recommended as a first choice in treating retinal detachments due to giant tears.
