We treated 7 eyes with retinal detachment due to giant tear and movable inverted posterior retinal flap. We applied modified scleral buckling and cryoretinopexy. Smoothly curved buckling along by the posterior edge of giant tear makes the tear close, because it prevents the retina from folding. Initial reattachment resulted in all 7 eyes, but 2 eye needed reoperation because of the recurrence of the retinal detachments. The above technique is easy and has few complication. It is recommended as a first choice in treating retinal detachments due to giant tears.
