無水晶体眼における網膜剥離の治療成績

Statistical analysis of aphakic retinal detachment

　1976年10月12日より1979年6月30日までの約2年9カ月の間に愛媛大学眼科に入院し，対網膜剥離手術を施行した無水晶体眼の網膜剥離24例28眼について統計的観察を行ない，次の結果が得られた。

　（1）同期間内で対網膜剥離手術を施行した特発性網膜剥離症例246例（260眼）の中で占める割合は9.8％（10.8％）で本邦における従来の報告例より高頻度にみられた。

　（2）性別は男79.2％，女20.8％で男性に多くみられた。

　（3）年齢別では50歳代が41.7％で最も多く平均年齢53.8歳であつた。

　（4）罹患側別では左眼に多くみられた。

　（5）近視の占める割合は42.9％で−6.0D以上の近視は32.1％であつた。

　（6）水晶体摘出後，網膜剥離発症までの期間は老人性白内障では1年未満のものが多く，53.8％と半数以上を占めた。

　（7）裂孔検出率（老人性白内障術後症例）は80.8％であつた。

　（8）主裂孔の形は弁状が61.9％で最も多く，大きさは1P．D．以下の小さな裂孔（80.9％）が多くみられた。裂孔の数では1個のものが多く57.1％を占めた。

　（9）主裂孔の経線位分布では耳上側に多く（61.9％），照準角分布では40°から60°の範囲に76.2％と圧倒的に多くみられた。

　（10）剥離の様態では胞状剥離が64.3％と最も多く，2象限以上の剥離は64.3％を占め全域剥離は28.6％にみられた。

An analysis was made on 28 eyes with aphakic retinal detachment treated by us during the period from October 1976 through June 1979. The incidence of retinal detachment in aphakic eyes comprised 10.8% (28 eyes) among all theidiopathic retinal detachments (260 eyes) during the period. This incidence is much higher than that hitherto reported in Japan and is lower than that overseas. There was a preponderance of males (79.2%) against females (20.8%). Aphakic detachment occurred more frequently during the sixth decade of life. The ages of the patients averaged 53.8 years.
