緒　言

　本案は眼瞼下垂に対して行われる眼瞼挙筋短縮法のうち，皮膚側から挙筋に達する手術法の改良に関するものである。

　従来，皮膚側から挙筋に達する方法は，眼輪筋などの組織の奥にある挙筋を見つけ出すのが難しいといわれており^1），手術中に誤つて挙筋を見失つてしまつた場合の対策も述べられている^2）。このような困難を解決するための方法も発表されている^3）が，私はそれよりも簡単な方法を考案したので報告する。

I devised a new technique to facilitate the levator resection through the skin. The purpose of the new technique is to avoid the danger to miss the levator during the surgical procedure. The surgeon everts the upper eyelid and main-tains it in the everted position. From the conjunc-tival side, three black 5-0 silk sutures are passed through the levator 1mm deep beneath the conjunctiva at the upper margin of thetarsus. They are the marking sutures which indicate the levator. The eyelid is replaced in its ordinary position. The surgeon incises the lid skin to the tarsus.
