緒　言

　SmithおよびRegan^1）が，1957年に，眼窩壁骨折の特殊な型として，blowout fractureという名称を初めて使い，その特異な発生機序を明らかにしてから21年経つた。眼窩骨折の中で最も多いのは，外壁すなわち頬骨の骨折である^2）が，blow-out fractureも決してまれなものではなく，日本でも，深道^3），久冨^2），三国^4）のら数多くの報告がある。

　blowout fractureは眼窩縁骨折の有無により，pure typeとimpure typeとに分けられているが，眼窩縁骨折のあるimpure typeをblowoutfractureに含めるかどうかについては，なお混乱している。Smith,Converse自身も，含めるか否かについては，定義が定まつていないようである^5）。

A 70-year-old male was seen due to severe contusion in the left eye region inflicted 2 weeks before. We observed blepharoptosis and severe lid swelling of the left eye. Visual acuity LE was reduced to light perception. When the lids were opened and inspected, the left bulbus was not observed in the conjunctival sac as the edema-tous conjunctiva was all that could be seen. High-voltage radiography and CT-scanning of the left orbit and the skull demonstrated moderate clou-diness of the left ethmoidal sinus and an obscure image of the left eyeball which seemed to be non-ruptured.
