特発性網膜前黄斑部線維症における大視症の臨床的意義

Macropsia as a new ocular symptom in idiopathic preretinal macular fibrosis

緒　言

　Wiseは^1），網膜内境界膜のwriklingを主要所見とする網膜前黄斑部線維症という疾患の概念を明らかにし，これには特発性と続発性のものがあることを述べた。この網膜前黄斑部線維症は網膜内境界膜付込が病変の場と考えられている比較的新しい黄斑部疾患として近年注目されるようになつた^2〜13）。

　本症の自覚症状としては，従来，視力障害，変視症，小視症，中心暗点，稀に光視症などがあげられているが，これらの発生機序についてはほとんど説明がなされず，しかも不明な点が多い。著者らは本症に大視症を訴えた症例を経験したので，以来注意して本症における大視症の有無を検査した結果，本症の自覚症状として従来全く記載のない大視症が，かなりの頻度に存在することを見いだした。

Ocular symptoms in some patients with idiopath-ic preretinal macular fibrosis (IPMF) have included mild visual disturbance, metamorphopsia, micro-psia and central scotoma. We recently noticed the presence of macropsia in the IPMF that has not previously been reported from an analysis of 47 patients with IPMF. The results obtained were as follows:

1) Macropsia was found in 18 out of 47 patients (38.3%). Significant difference was not found in visual acuity between the group with and without macropsia.

2) Grade of IPMF revealed significant differences between the group with and without macropsia.
