緒　言

　網膜［剥］離手術に対するscleral bucklingにおけるPlombe縫着の方向は　radialとcircum-ferentialとに2大別される。最近まで広く行なわれているのはcircumferential bucklingである。近年radial bucklingの効用を主張したLincoff （1965）の報告にはじまり，1975年には，同氏等が数学的解析を行ないradial bucklingの方がより望ましいと望べている。我々は後極部を，周辺部網膜にある裂孔よりblockするという目的を第一義的に考え，現在までcircumfer-ential bucklingを行なつてきたが，その裂孔の形，大きさ，位置および剥離の状態によつてはradial bucklingの方が望ましく，また，深部裂孔においては技術的にも本法による操作が比較的容易である事より，今回3例にradial bucklingのみを行ない，9例にcircumferential buckli-ngにradial bucklingを併用し，全例ともに復位に成功し，術後6カ月間以上にわたり経過を観察しているが，認むべき合併症もみられなかつたので，今回報告する。

We operated 11 eyes with various types of rhegmatogenous retinal detachment by means of radially placed scleral buckling, T-type bu-ckling, inverted T-type buckling or U-type bu-ckling as an independent procedure or as supp-lement to circumferential buckling surgery. 3mm silicone plombe was used as the implanta-tion material throughout. The surgical outcome was successful in all the cases. Also, the po-stoperative course was virtually uneventful.
