義眼装用の為の鞏膜後半部切除術

Excision of Rear Parts of Sclera to put on Artificial-Eye

　眼球摘出又は眼球内容除去後，義眼装入により，其の外観が健眼と全く同じく，其上義眼の運動が健眼と大差なき様企つるを以て理想とする。斯かる目的を達するために，眼科医は古くより多様の術式や義眼台の考案を試みつゝあるも，今尚満足すべき方法がなき様である。著者は眼球摘出を可成避けて，代りに眼球内容を除去し，眼球後剖の鞏膜を部分的に切除（2〜3個所に穴を作る）したる後出来得る限り大なるプラスチツク球を鞏膜腔内に挿入し，薄きプラスチツク製義眼所謂一重義眼を装用することにより，上記の如き目的を達せんと試みた。其の結果は鞏角膜葡萄腫の如き鞏膜内腔の拡大箸明なる症例に於ては満足なる結果を得た。然し正常大の鞏膜内腔を有する症例の大多数に於ては挿入する義眼台（プラスチツク球）が大なるため，術後数箇月後に，鞏膜の萎縮により，義眼台が角膜切除部より脱出するのが普通である。

After usual Exenteration, learing four muscli rectus and sclera attached to them like a ring, all rest part of sclera is taken out. A plastic ball, about 20mm in dia, is inserted through the space between the m.rectus nasalis and the m.rectus inferior into the orbit. Then the sclera and Tenon's membrane are sutured with cot gut and the sclera is sutured with silkworm gut. This method give far better results than any other method. I have experienced.
