鞏膜切除短縮術の1感染例について

A case of infection following scleral resection

　（1）網膜剥離に対する鞏膜切除短縮術に際して，ペニシリン並にスルフアミン耐性をもつ溶血性黄色ブドウ球菌の感染が成立し，手術野鞏膜の化膿性壊死を来たし，続いて炎性滲出に基く広汎な脈絡膜剥離を招来したが，起炎菌が耐性を有しない抗菌性物質の強力投与と，ACTH持続点滴による炎性反応阻止との結果，幸に消炎，漸くにして網膜剥離治療目的を達成し得た1症例を報告した。

　（2）最近の4年間に鞏膜切除短縮術を行つた48網膜剥離眼について，術後経過の異常を調査して，本報の1感染例の外に，非化膿性炎症による鞏膜壊死及び短縮創痕の伸展を呈した1眼と，糸状角膜炎の高頻度（20.8％）の併発を認めた。

　The case : 38 year old male. Vision of the right eye had been lost 11 years previously due to retinal detachment.

　Three monthes after being struck in the left eye by a football, the patient came to this clinic complaining of a visual field defect of the nasal side. R. V. =⦵, L. V. =n. d. (0.03× -15D). Examination of the left fundus revealed a flat detachment of the temporal half of the retina with eight flap tears in the temporal peripheral region. Surgery consisting of a combination of tear closing diathermy and lamellar scleral resection was performed.
